D/C's don't all happen the same way. If there is an actual glitch on the gaming server (like a process just dies), and you get booted out immediately, then you pretty much will luck out and get kicked quickly back to the main menu after seeing a "network disconnect" box on your screen. Sometimes you'll get completely d/c'd from even the chat server where you will need to log back in again.
If you see weirdness with the chat server while in-game, it is often times a good idea to portal back to town (if not already there), and leave the game as that is usually a sign that something is going on. When you first login to the main screen, you are basically first logging into the chat server. From there, you initiate a connection to the gaming servers (resume/start game). If the chat server goes to pot, so does your gaming session.
If your disconnect is somehow related to a network issue upstream from that gaming server which is the most common one (like say one of the routers line card nodes went down on AT&T's network, and you disconnected during the hotfail switchover, or say there was an issue closer to your ISP), the outcome will depend:
- if you had aggro'd any monsters and your final commands weren't able to get your character to disengage any of those mobs, then your character will eventually be standing in game since it currently can take the gaming server over a minute to acknowledge the client has actually disconnected. Only once it has registered a disconnect, will it then start the 10 second countdown to remove the character from the game.
- if you didn't have any monsters engaged and didn't aggro any (either your character or follower), you then have a great chance at seeing your character alive.
- sometimes you may not see any stray mobs in your field of view but aggro them (or possibly your follower). It's why I always clear a path and when encountering lag, I beeline back exactly the way I came from especially when the last video frame shows I hadn't actually engaged yet (once engaged in combat like you were, it is better to continue attacking since that will be the last set of commands that will be seen by the server anyway). Likewise, your follower could also be warding off and distracting a mob while you are disconnected (buying the time needed).
Bottomline, it really depends and can come down to luck. It's why some try to build for d/c's with life regen, thorns, and skills that can deal out damage while AFK (no guarantees either but potentially increases the chances of surviving lags/disconnects). It's also why some will only play in groups. Note that if its a gaming server issue, everyone will see issues at the same time. If there is a networking problem on AT&T's network (normally would be much rarer than issues from outside their network back to ones ISP), then everyone in the group will also see increased latency and no one will be able to save each others bacon. Most times though, the network problems leading to higher latency are relative to each player which is why group play normally increases the chances of survivability since most times, others will be able to help ward off a mob until the player who is lagged/disconnected is removed from the game by the server.
